At Holly Springs Baptist Church we desire to honor your wishes regarding photos and videos of you that we post on the Internet or other promotional material.

By completing the form and submitting to the church you agree to the following:
I/We understand that my/our child’s likeness may be photographed or videotaped by Holly Springs Baptist Church in the course of church activities. I/We hereby give consent for the church to use my/our child’s photos or likeness in promotional materials.
